A brown or grey patch on the chimney breast, upstairs or down, is the classic sign. The brickwork above has become saturated and moisture is wicking through the plaster.
If a ceiling mark darkens during a downpour and dries out afterwards, rain is getting past the flaunching or flashing and running down inside the stack.
A damp, musty smell around the fireplace often means water is standing inside the flue, usually because an open, unused pot has no cap on it.
Bits of mortar or brick dust dropping into the fireplace mean the joints or flaunching above are breaking up and falling down the flue.
Wallpaper lifting or paint bubbling on walls beside the chimney is moisture pushing out through the plaster from saturated brickwork behind.
Stand back and sight up the stack. Any lean, gap or stepped cracking in the brickwork needs looking at promptly, before wind or frost makes it dangerous.

Not every chimney is worth saving. Where the fireplace was blocked up years ago and the stack is now just weight, maintenance and a leak waiting to happen, taking it down is often the sensible answer. We remove redundant stacks safely, working down course by course rather than by force, with the debris cleared as we go and the rest of the roof protected while we work.
The hole left behind is the part that matters. We deck the opening, felt and batten it, then tile or slate it back so it reads as part of the roof rather than a patch, and we make good the loft space or the room below where the stack passed through.
